WHAT WE DO

Care beyond a
single check-up.

From the first screening to the next step, our services bring structure and continuity to preventive care.

01

Health Screening Services

Understand your health. Take the next step.

Regular, non-invasive screening brings together physical examinations, body measurements and a review of health concerns. Findings are documented so individuals can understand their results and receive recommendations for further assessment where needed.

  • General physical examination and body measurements
  • Review of symptoms and relevant health history
  • Individual findings, recommendations and referrals
02

Health Care Management Services

Connect screening with coordinated care.

Care management helps turn screening findings into an organised plan for follow-up. Individual health profiles, practical recommendations and targeted referrals provide a clearer path between identifying a concern and seeking appropriate clinical support.

  • Individual health profiles and record updates
  • Coordination of recommendations and referrals
  • Follow-up planning based on identified needs
03

Health Monitoring and Evaluation Services

Follow progress. Inform better decisions.

Ongoing monitoring brings screening records together over time, helping teams review changes and identify follow-up needs. Periodic reporting gives authorised stakeholders a structured view of programme activity and areas requiring attention.

  • Periodic review of screening observations
  • Electronic records with specified stakeholder access
  • Monthly reporting and programme review
04

Health Promotion Services

Make prevention part of everyday life.

Community awareness initiatives encourage people to take an active interest in their health. Outreach activities and preventive health campaigns create opportunities to discuss healthy habits, the value of screening and when to seek professional advice.

  • Community health awareness campaigns
  • Outreach and preventive care initiatives
  • Practical messages that encourage timely follow-up
05

Health Education Services

Build knowledge. Support informed choices.

Health education sessions help communities, parents and organisations understand preventive care. Talks and workshops explain screening, common health concerns and recommended next steps in accessible language, with topics tailored to the audience.

  • Interactive health talks and workshops
  • Parent-focused sessions on prevention
  • Audience-specific preventive health education

A LITTLE MORE CLARITY

Your questions,
answered.

What does a screening include?

The PHCS process includes a general physical examination, anthropometry (body measurements), a review of symptoms, documentation and recommendations. The team can explain the planned scope before a programme begins.

Is the screening invasive?

The initial screening process is non-invasive. Where a finding warrants further assessment, a clinician may recommend additional investigations through a targeted referral.

Who can view screening records?

The existing PHCS service describes specified access for predefined stakeholders. Contact the team to confirm who will receive information and how records will be handled for your programme.

How do we arrange a programme?

Call PHCS to discuss your community, location, participant numbers and goals. The team can confirm the scope, schedule and arrangements appropriate to your requirements.

Does screening replace a medical consultation?

Screening can identify concerns that need further assessment. It does not establish every diagnosis or replace individual medical care. Discuss symptoms and recommended follow-up with a qualified clinician.
